To open an account with Cash/Credit Card terms:  

 

We must have the following:

1.    Account Application

2.    Copy of State issued Resale Certificate

3.    Completed State Resale Form: Please click here to find your state(s) form.  The Resale form is required in any state that has sales tax. Please sign, date and write in your Resale Number.

4.    Credit Card Authorization (optional)

To open an account and apply for Credit terms: 

 

We must have the following:

1.    Credit Application

2.    Electronic Check (Chax) Form (optional for credit terms customers)

3.    Copy of State issued Resale Certificate
